File Uploads Failing in Assistant & Responses API

I try uploading files (PDFs, small/big, doesn’t matter) in the “Attach files to vector store” popup and it looks like they upload just fine—shows “uploaded” next to each file.

But as soon as I check the thread’s files, there’s a big red dot next to “File search.” When I click into it, I see that every single file actually failed with “Failed” after indexing. This issue occurs for both the Assistant API and Responses API, regardless of the type of PDF used.
