I’ve already received “Failed to sample tokens” error from Codex web version several times now, and the most disappointing thing is that I can’t recover from the last Codex action. I need to retry the request entirely

i’m getting this error as well it’s getting me seriously stuck. seems to be related to session length. it kills the session instead of making a commit and push. i do realize it’s my work for as long as you can instruction encouraging this behavior, but it was working before the update.

I confirm the same problem. Furthermore, now everything is slow, almost unusable.

Same here, any session over 30 minutes and it fails with that error.
Pretty frustrating when it happens right at the end before creating the PR..

Getting this as well on every cloud request. Same request works in local-mode.
It doesn’t seem duration or complexity-related for me. I’ll kick off 4 variants and all 4 will complete in <5 minutes all with the same Failed to sample tokens message. This happens with longer running tasks as well. Requested a much more complex query and all 4 took about an hour to complete and resulted in same Failed to sample tokens message.
Looking at the logs on each of these, it seems like it’s happening when Codex is wrapping up the task and waiting for pre-commit checks to finish.
I was not experiencing this issue pre gpt5-codex update.

Face the same issue here.
I asked the AI to create small tasks and I’m running small tasks instead of a bigger one, but it’s taking 10 times more time than it was before.

I get this issue all the time. I always need to retry and ask for it to split the task into 2 or more parts.
Before the update I ran into an issue like this very very rarely.

After some experimentation on my end, it seems almost exclusively due to the duration (and likely large output) of my pre-commit checks.
I had tasks that would run <5mins that would fail on pre-commit checks and also had a few tasks that ran for over an hour that only failed once getting to pre-commit checks. I’ve tweaked my checks and moved some items from pre-commit to pre-push to reduce pre-commit duration/output and Codex Cloud tasks are finishing successfully now.
Hope that helps others - look for long-running/high-output commands!

Yeah, here is were a more descriptive error message would help. A lot of folks got to try CODEX when it was really bad and have no clue, right now is a pleasure to use again.
Well thanks to people like yourself that kept poking your head in, the rest of us know it’s safe now.
I’ll make a few web pages with it later now that I know the waters fine again…
