Codex Failing to Create New Tasks and Follow-Ups

EDIT: Resolved for me. One user says not resolved for them yet. However it is working as normal again for me now.

Tried removing GitHub in connectors and re-adding (as this had fixed an issue yesterday and got it working again).

However, now codex just refuses to do any “Ask” or “Code” tasks. Says either “Failed to Create Task” or “Failed to Create Follow-Up”. It also does not let me edit the Environment at all. Hitting Save shows an error of “Failed to Update Environment”.

I have only done one task today, so I doubt I’ve hit a daily limit of any sort. I wonder, is there a monthly limit I may have hit? Or weekly?

Otherwise, there seems to be an issue right now…

Same here. the issue lasted 1 hour. Now everything worked fine again.

@thierry1 I tested just now and mine’s back to working aswell!

I’m not sure the etiquette of these threads and whether I should delete my post now that it’s resolved… I suppose I’ll keep it up until told otherwise, at least so that people who are still experiencing this issue, like @allstat , can make it known.

Keep the message on the board. This has happened to me several times over the last month and it’s best it the issue is documented. Just bad error handling, hopefully we get some nice message in the future about an outage instead of left scratching our heads wondering what we have done wrong…

Happening to me today too. Error message is “Failed to create follow-up”

It is currently happening to me right now, I was running two tasks at the time and when starting the third I was getting the “failed to create task” message. Are we limited in the amount of tasks we can execute concurrently and or at specific time intervals?

I can confirm that codex is failing to create pull requests currently. Otherwise I like codex very much. I would be interested in trying special new features or “beta” testing, please contact me if that is something the development team would find beneficial.

Codex is not able to create any task or update anything.

There’s an error in console:

Seems like their server is down?

Its working Now ! I think its was 30 min Isssue

I’m getting “Failed to create task” 2 days in a row. Tried to clear cache, delete GitHub connection, verified my “organization” (which is my self) on platform.openai - nothing helps. Tried both on Mac and iPhone. I wanted to try Codex for the first time yesterday, but still can’t do it :frowning:

Turned out, I wasn’t able to crate environment for empty git repo :man_facepalming:
added readme, and everything worked :sweat_smile:

getting the Failed to create task error also now

For the past week, I’ve been battling a persistent error when trying to use Codex:

“Failed to create task.”

No matter what I attempted, writing code, submitting a prompt, or even basic interaction, the same message appeared every single time. The platform was completely non-functional.

Troubleshooting Steps Taken:

  • Logged in/out
  • Cleared cookies and cache
  • Switched machines
  • Reconnected GitHub
  • Verified network/firewall
  • Checked system time/settings

None of it worked. I was stuck with a bricked Codex experience.

Update:

After additional testing, I’ve now isolated the issue to the Microsoft Edge browser. I switched over to Opera, and Codex works flawlessly, no errors, no hiccups.

This suggests a browser-specific compatibility issue affecting some Edge users.

Is there anyone else experiencing this issue?
