Hi, we’re seeing intermittent duplicate responses, like the model responding with the exact same message twice. Has anyone else seen something similar for gpt-5.4?

Could you please share the steps to reproduce this issue, including the API request you made? Based on the screenshot shared by @Abrie_Cronje, it appears this may be occurring in the Chat Completions API.
