Hi, this started happening recently, maybe a day or two ago, and it’s specifically happening with the 4o model. No matter how different I instructed it, nothing seems to work without triggering a creation of a document. The mini version also experienced it, but it seems to have fixed itself. However, now it deletes memories completely instead of updating them.
