Bug in macOS Desktop App GUI

Subject: ChatGPT macOS 26.924.22138 — window manager hides main window & title-bar hit-test is vertically offset (macOS Tahoe 26.7)

Hi! I’m reporting a reproducible window-management bug in the ChatGPT macOS desktop app.

**Environment:** ChatGPT for macOS v26.924.22138 (Codex Framework 154.0.8037.57), macOS 26.7 Tahoe (25G229), MacBook Pro built-in display only (1800×1169 pts, 3024×1964 Retina). Bug started after the 2026-09-28 14:40 auto-update, on the first launch after a 14:57 reboot.

**Symptoms:**

1. With the window **restored** (not maximized), the traffic-light buttons only respond to clicks in a **~39 px-tall strip at the very top**; the rest of the button area is dead. **Maximizing the window fixes it** (buttons clickable across their full area).

2. After opening, the main window self-maximizes (~1 s), then **at ~10 s it animates itself off-screen** (measured x=-1364), and **at ~14 s it reappears** at a persisted ghost frame from a previous display configuration (1128×2229 at y=-390/-55; height ≈ 2× the display).

3. When this happens the app’s **accessibility tree is empty** (System Events can’t see window 1) even though windows are on screen.

**Evidence (live CGWindowList sampling):** a persistent **off-screen window 1128×2229 at (993,-55), layer 3** coexists with the visible window. With the Avatar Overlay (“Classmates”) disabled and its persisted state removed, the main window stays stable and clickable (30+ min verified); re-enabling the pet brings the hide-away choreography back. However, the hit-test offset persists even with the pet disabled, and writing a correct overlay frame into the persisted daemon state (`~/.codex/.codex-global-state.json`, keys `electron-avatar-overlay-bounds` with orphaned `displayId`/`byResolution` entries) is **ignored by the app** — the corrupt frame is generated internally, not read from persisted state. Chrome profile shows `exit_type: “Crashed”`.

**Ruled out:** TCC/permissions (no folder grants; drag & drop handled by Finder), third-party event taps/window managers (none running during first occurrence), stale daemon state alone (purging it fixes the hidden window but not the hit-test).

**Workaround:** keep the window maximized.

**Request:** please review attached-launcher/avatar-overlay frame handling after display-configuration changes on single-display Macs, and the title-bar hit-test ↔ content desync in restored windows.

Thanks!
