- Notice the first thing it thinks about
- This happened twice at least and the topics are random

I agree that from what I see there is no reason to expect that in the thinking process.
While I can not answer your question, would it be possible for you to share a link to the chat so that we can review all of it. Also that might get the attention of the OpenAI staff.
