Urgent Codex CLI maintainer triage requested: two reproducible workflow-blocking reliability failures in 0.153.4

I am urgently asking for help getting two serious Codex CLI reliability issues in front of the appropriate Codex engineering owners.

I have already reported both issues in the official openai/codex repository, collected diagnostics, inspected the relevant client source, and uploaded additional diagnostics through Codex /feedback. I am posting here because both reports are currently still unassigned and have received no maintainer response, while the failures are severely disrupting real development work.

I am a ChatGPT Pro user and I am not reporting a hypothetical edge case or a generic “my network disconnected” problem. One of the failures now has a deterministic localhost-only reproduction against the actual installed Codex CLI 0.153.4 Windows binary.

1. WebSocket false-dead-stream / idle watchdog failure

GitHub:

This issue has been reduced to a controlled three-case experiment using the unmodified installed Codex CLI executable and a synthetic server listening only on localhost.

There is:

  • no OpenAI inference request;
  • no account credential involved;
  • no image payload;
  • no production proxy in the model-request path.

In the failing case, the peer continuously sends WebSocket protocol Ping frames and Codex continuously returns valid Pong frames.

Nevertheless, Codex still reports:

idle timeout waiting for websocket

and abandons the WebSocket / falls back.

The last valid Pong was observed only about 47 ms before the fallback occurred.

Two controls are particularly important:

  • replacing protocol Ping with an application-level JSON heartbeat allows the identical delayed response to complete normally over WebSocket;
  • changing only the configured idle threshold also allows the identical response to complete normally.

This appears to demonstrate a real distinction between transport liveness and forwarded Responses application-event silence in the current idle-watchdog path.

I am not claiming that Ping/Pong proves that a model is making useful inference progress. The claim is narrower: a live transport and application silence are currently being collapsed into a failure reported as idle timeout waiting for websocket, which can cause a viable long-reasoning request to be abandoned and retried.

In production this has manifested as repeated multi-minute stalls and retry/fallback cycles during long Codex reasoning.

2. Aggregate image-history serialized-request amplification

GitHub:

A separate affected image-assisted session produced a measured request of:

63,759,837 bytes before compression

and

45,853,691 bytes after zstd compression

The active history contained 38 typed image occurrences accounting for about 61.28 million data-URL characters.

Important details:

  • zero compaction records existed at the measured failure snapshot;
  • 37 of the 38 image data URLs were distinct;
  • exact deduplication would remove only about 1.83% of the image data;
  • no individual image exceeded 10 MiB;
  • the largest individual data URL was about 5.18 MB.

So this is not adequately addressed by simply deduplicating identical images or imposing only a 10 MiB per-image ceiling.

The concerning architectural boundary appears to be that model-context/token budgeting and actual serialized request byte growth can diverge substantially during an ordinary multimodal working window.

A later successful compaction reduced the retained embedded image bytes by about 93%, after which productive work resumed. I am not claiming this proves that every observed stall was caused solely by payload size, but it is strong evidence that active-history serialized size is an important contributor.

Diagnostics already provided

I also uploaded additional diagnostics through Codex /feedback.

Uploaded thread:

Diagnostic bridge:

A current codex doctor --json run reports, among other things:

  • Codex CLI 0.153.4;
  • Windows x64;
  • ChatGPT authentication;
  • active provider HTTP reachability succeeding;
  • Responses WebSocket handshake succeeding with HTTP 101.

That does not prove every production connection is healthy, but it makes a simple “endpoint is unreachable” explanation insufficient for the controlled failure above.

What I am asking for

I would sincerely appreciate help from anyone who can route these reports to the relevant Codex engineering owners.

For #43022, I am asking for triage by the owner of the Responses WebSocket transport / retry / idle-watchdog path.

For #43015, I am asking for triage by the owner of image preparation, conversation history, context management, and request serialization.

I am especially looking for:

  1. confirmation that the issues have reached the appropriate engineering team;
  2. a supported short-term mitigation for affected users;
  3. clarification on whether fixes are already being developed;
  4. identification of the release that will contain a correction, if known.

These issues are currently making some long-running Codex development workflows extremely difficult to use reliably.

I understand that the repository has a very large issue backlog and that today may not be a normal engineering workday. I am not asking for special treatment without evidence. I am asking for urgent triage because substantial reproduction work has already been done, the user impact is severe, and the reports now contain enough technical detail for an engineer to begin investigating without first having to reconstruct the problem from scratch.

If anyone from OpenAI or the Codex team sees this, I would be very grateful if you could help route #43022 and #43015 to the appropriate maintainers.

Thank you.

Please, please help me. I really don’t know what to do now, and I’ve already tried everything I could, but I still can’t solve this problem on my own. Your help is extremely important to me. Even if you can only offer some advice, information, or whatever support you’re able to provide, I would be immensely grateful. If you need to know more about the situation, I’m willing to explain everything honestly. I sincerely ask you not to ignore my request. Please.

Follow-up — September 8, 2026

Hello Codex team — apologies for one brief follow-up, and thank you to anyone who can help with routing.

I have not yet seen a public maintainer response on the two GitHub reports. I appreciate that they were filed recently, and I am not asking anyone to commit to an immediate fix or release date. The documented failures have substantially disrupted my development work, so even a brief indication of the appropriate team or a supported mitigation would be very helpful.

The two canonical reports are:

  • #43022 — a controlled localhost reproduction using the installed Windows 0.153.4 binary, showing the distinction between ongoing protocol Ping/Pong activity and the application-message idle timeout.
  • #43015— measured aggregate image-history growth producing a 63.8 MB request before compaction, with the environment, recovery observations, and causality limits documented.

Additional diagnostics were submitted through Codex /feedback, with the reference recorded in #43168:
01a0755c-377e-75a1-9085-6f6a34bda5cf

Could someone from the relevant team please help route these reports, or point me to an existing tracker or supported recovery procedure?

I am keeping the technical evidence in the two GitHub issues so that this community thread does not become a separate investigation.

Thank you very much for your time and assistance.
