Task list read fails with nested ERROR while the outer result says Action completed

Environment
ChatGPT task-management tool, mcp__codex_apps__automations_list, called with the exact arguments {}. This is the built-in task-list operation, not an application using the OpenAI API. The client build, selected model and backend version for the failed calls were not recorded.

Observed sequence and timing
Request a read-only list of existing automations. Two calls failed on October 14, 2026:

  • 20:13:05-20:13:11 UTC.
  • 21:04:42-21:04:46 UTC.

Both returned “Action completed.” and outer isError:false, while structuredContent.structuredContent.status was ERROR. Relevant returned fields:

status: ERROR
message begins: The task service returned an unexpected error.
user_message: Unable to complete task request. Try again.

No task list was returned. This prevented verification of an existing reminder’s saved schedule and enabled state.

Expected / actual
A read should return the task list or an unambiguous failure. Here, the success wrapper contradicts the nested failure. Please propagate the operation error consistently and provide a supported read-only way to verify task state.

I confirmed on October 19 that the issue remains relevant; these timestamps still refer to the original two calls. They do not prove that any reminder stopped, a schedule was lost or an earlier update failed. No task mutations or duplicate tasks were used for these checks. Request/trace IDs and an HTTP status were absent.
