OpenAI finetuning continuously failing

Why is the fine-tuning failing continuously and encountering an internal server error? Why is this happening?

Fine-tuning jobs failing with "internal error"

This problem has returned after temporarily working again last week (see: Internal Error during fine-tuning). I’m using a fine-tuning process that has worked successfully hundreds of times over the past several years, but now fails. I am getting the same thing I’m fine-tuning gpt-4.1-mini. When I started getting this on new fine-tuning jobs I went back and tried to re-run a job that succeeded two days ago. It also failed, so this is very reproducible. Let me know if I can do anything to help trouble-shoot. I also adjusted the budget on my account, but these are pretty small jobs (usually finish in under 15 minutes), and increasing the budget limits had no effect.
