My GPT keeps randomly picking a document in its knowledge section to summarise, rather than the one I attach

I use projects with uploaded documents, as well as custom instructions that explicitly forbid hallucination and making up information. Yet, mine STILL hallucinates all the time. I tell it to quote a document and it makes up citations. It’ll “quote” the document with made up nonsense. Then I have to find what I wanted it to quote and paste it into the chat. Then it’s like, “Oops, my bad.” It’s really frustrating.
