Category: Bugs
Report type: Cross-platform bug report and community investigation
Reporting context: OpenAI beta tester
Observation period: October 2026
Confirmed Android application: ChatGPT for Android 1.2026.293 (19)
1. Overview
I am documenting a series of recurring issues affecting my use of ChatGPT across multiple areas of the product.
These include semantic drift, memory discontinuity, loss of established project context, unreliable Library retrieval, conversation fragmentation, Memory Summary loading errors, reference-image loading failures, image-generation requests becoming stuck, intermittent web authentication failures and the generation of information or entities that do not exist.
I have experienced these issues personally and have encountered reports from other users describing overlapping symptoms across the OpenAI Community and Reddit.
The purpose of this report is to establish whether these problems represent separate, unrelated regressions or potentially interconnected failures affecting broader aspects of ChatGPT’s functionality.
I am an OpenAI beta tester and am approaching this as an observational and reproducibility exercise.
2. Environment
- Android application: ChatGPT for Android 1.2026.293 (19).
- Operating system: Android. Exact version to be confirmed.
- Device: To be confirmed.
- Web environment: Browser-based ChatGPT. Browser and version to be confirmed.
- Model: Not consistently identifiable from the available interface. Specific model versions are therefore not attributed to individual incidents unless independently established.
3. Observed issues
A. Semantic drift and contextual substitution
ChatGPT sometimes changes the meaning of established references, misinterprets context or substitutes an entirely different interpretation while continuing the conversation as though the interpretation were correct.
In vivo example from this report:
I referred to Survivor, the television production. ChatGPT interpreted this as survival in the existential or personal sense and constructed an explanation around that incorrect interpretation.
This illustrates contextual referent substitution followed by continuation based on an incorrect interpretation.
B. Memory discontinuity
Previously established information is inconsistently retained or retrieved.
Observed behaviour includes:
- Loss of previously established conversational context.
- Failure to retrieve information available in previous conversations or saved context.
- Inconsistent retrieval of established information.
- Deterioration in continuity after periods of inactivity.
- Repeated reconstruction of information and instructions.
One particularly noticeable behaviour has been the loss of conversational continuity after approximately 30 minutes of inactivity. Returning to an established conversation and contributing only one or two sentences can sometimes be sufficient for the assistant to lose track of the preceding discussion.
This is not simply a matter of conversation length.
C. Project comprehension and continuity
Long-running projects have become increasingly difficult to maintain.
Previously established project structures, decisions, relationships, terminology, canon and historical context are inconsistently understood or retrieved.
In some instances, the assistant appears unable to distinguish established project information from newly introduced material.
This affects ongoing creative and professional workflows with substantial historical documentation.
Projects that previously functioned successfully can become difficult to continue without extensive reconstruction and repeated clarification.
D. Library retrieval failures
I have encountered repeated difficulties retrieving relevant material from the ChatGPT Library.
One recurring response has been:
“I cannot reliably retrieve the context from the library, but we’ll continue searching.”
The concern is not simply that retrieval sometimes fails. It is also that the assistant may continue a workflow without establishing whether the necessary source material has actually been recovered.
This creates uncertainty about the completeness and accuracy of subsequent work.
E. Memory Summary loading failures
When accessing the Memory Summary, I sometimes encounter a loading error.
This affects access to the memory interface itself, rather than only the assistant’s ability to retrieve information during a conversation.
I would like to establish whether other users are experiencing similar failures and whether these correlate with broader memory or account-level issues.
F. Reference-image loading failures
My saved reference image sometimes fails to load when I access it through ChatGPT.
This has occurred when using the Android application and web interface.
Importantly, I am not claiming that the reference image has been deleted or removed from the account. The observed behaviour is that the image does not load when I attempt to access it.
I cannot currently establish whether this is an image-loading problem, a retrieval failure or an issue with the association between the saved reference image and the account.
G. Conversation fragmentation
Threads sometimes appear to break into separate pieces, after which conversational context becomes unavailable or effectively lost.
This is distinct from ordinary context-window limitations.
The concern is the integrity and continuity of an existing conversation, including whether its constituent messages and established context remain accessible and connected.
H. Fabrication of information and non-existent entities
I have experienced instances in which ChatGPT generates information, references, features, sources, project details or other material that does not actually exist.
Observed behaviour includes:
- Generation of apparently factual but nonexistent information.
- Introduction of details never established in the conversation.
- References to nonexistent project elements or undocumented material.
- Confident continuation based on fabricated information rather than acknowledgement of uncertainty.
It is important to distinguish ordinary factual errors from fabrication occurring alongside continuity failures.
Invented details can become incorporated into subsequent project work, potentially compounding the original error.
I am also interested in whether the assistant recognises and corrects these fabrications when challenged.
I. Image-generation failures
Image-generation requests sometimes become stuck or fail to complete as expected.
I would like to establish whether this occurs across different prompts, conversations, platforms and models.
J. Web authentication and access
I have experienced periods lasting several hours during which I have been unable to log in through the ChatGPT web browser.
I have also encountered unexpected session interruptions and logouts.
Other users have reported similar authentication and access difficulties.
I would like to establish whether these are isolated authentication incidents or potentially related to wider service reliability problems.
K. Communication and incident transparency
I have not found sufficiently clear public communication establishing whether these problems are being investigated together, separately, or whether they are recognised as related incidents.
I appreciate that individual reports do not necessarily indicate a platform-wide problem.
However, clearer communication about known incidents, investigations and potential regressions would help users understand what they are experiencing.
4. Reproducibility and testing
As an OpenAI beta tester, I am particularly interested in reproducibility.
I have encountered recurring instances of these behaviours across different conversations and product surfaces.
Some observations have been sufficiently repeatable to identify recurring patterns, although I have not established a universal reproduction procedure for every issue.
Examples include:
- Returning to an established conversation after a period of inactivity and observing substantial context loss.
- Attempting to continue an established project and finding that previously established information is inconsistently understood or retrieved.
- Requesting information from the Library and receiving acknowledgements that retrieval is unreliable.
- Observing contextual substitution within an otherwise functioning conversation.
- Accessing the Memory Summary and encountering a loading error.
- Attempting to access a saved reference image and finding that it does not load.
- Encountering fragmented conversation threads followed by loss of context.
- Observing image-generation requests that do not complete.
- Experiencing extended periods of web authentication failure.
- Identifying fabricated information or references to nonexistent entities in generated responses.
I would welcome suggestions for additional reproducibility tests that could help isolate individual failure modes.
5. Independent community observations
I have encountered reports across the OpenAI Community and Reddit describing overlapping issues.
These include:
- Loss of context within existing conversations.
- Memory inconsistencies and retrieval failures.
- Changes in established response behaviour.
- Project continuity problems.
- Android session and history issues.
- Unexpected logouts and authentication difficulties.
- Image-generation problems.
I have also received direct messages from other users describing experiences that appear similar to mine.
These reports are not necessarily evidence of a shared technical cause. They are worth documenting alongside firsthand observations.
6. Methodological note: Avoiding confirmation bias
I want to be explicit about the purpose and limitations of this report.
I am not attempting to establish a predetermined conclusion about the cause, scope or relationship of these problems.
I recognise the possibility of confirmation bias, particularly when multiple users report apparently similar symptoms.
Similar symptoms do not necessarily establish a shared technical cause.
Equally, the possibility that these failures are unrelated should remain open.
I am distinguishing between:
- Firsthand observations: Issues I have personally experienced.
- Independent reports: Experiences described by other users.
- Reproducible behaviours: Issues that recur under identifiable circumstances.
- Unverified hypotheses: Potential relationships between observed failures.
I also want to document reports of improvement with the same scrutiny as reports of deterioration.
If users are experiencing greater stability, partial recovery or resolution of particular issues, that information is equally relevant.
The purpose is to collect evidence, establish a chronology and test possible explanations.
I am not seeking confirmation of an existing theory. I am seeking evidence that allows the theory to be tested.
7. Questions for the community
I would particularly appreciate feedback from users who have experienced any of the following:
- Has conversational continuity deteriorated, particularly after periods of inactivity?
- Are you experiencing semantic drift or contextual substitution?
- Has memory retrieval become less consistent?
- Are long-running projects becoming increasingly difficult to maintain?
- Have you encountered unreliable Library searches?
- Does your Memory Summary sometimes fail to load?
- Have saved reference images failed to load?
- Have existing conversation threads fragmented or lost context?
- Are image-generation requests becoming stuck?
- Have you experienced prolonged web authentication failures?
- Have you encountered fabricated information or references to nonexistent entities?
- Have you noticed any recent improvement, stabilisation or further deterioration?
Where possible, please include your platform, device, operating system, model, approximate dates and examples.
8. Questions for OpenAI
I would appreciate clarification on the following:
- Are these reported behaviours recognised issues?
- Are continuity, memory retrieval and project-context failures being investigated?
- Are there known regressions affecting particular platforms or model versions?
- Are Memory Summary loading failures and reference-image loading problems known?
- Are image-generation and web authentication failures being investigated separately?
- Is there any known relationship between these incidents?
- Is there a public incident or status communication users can follow?
9. Privacy and supporting evidence
I am documenting these observations without publishing private conversations, personal information, saved reference images or screenshots containing sensitive material.
I can provide written descriptions, application-version information, approximate dates, error messages and reproducible examples.
I believe these are sufficient to begin a community discussion without unnecessarily exposing private user data.
10. Closing
ChatGPT is used for considerably more than isolated questions and answers. Many users depend on continuity across long-running conversations, research, creative projects and professional workflows.
When continuity, retrieval, data accessibility, authentication and other core functions become unreliable, the consequences extend beyond individual failed interactions.
I am documenting these observations because I would like to understand what is happening, establish whether the problems are reproducible and contribute useful information to the community.
I welcome contradictory evidence, reports of improvement and alternative explanations.
The central question is straightforward: Are we observing several independent product regressions, or potentially interconnected symptoms of a broader continuity and service-reliability problem?
I would like to investigate that question with the community rather than assume an answer.
