I’m running into this issue frequently, but only over the past month or so…
What happens is I write and submit a prompt, and get my response back, but the prompt bubble is missing. For example:

I have had this exact problem several times on Windows 11 during the past few weeks. Today a prompt disappeared which was the fork for a second thread within the convo session (the 1/2, 2/2 links), and these also disappeared. As a result I can no longer access the 2nd thread.
