Custom GPT used to work, now hallucinates and does not use specific data from files as instructed

I’m having the exact same problem.

I have a TXT file around 20 pages. It just completely completely ignores it sometimes, but other times it works perfectly. On the times it gives bad unrelated answers, It even claims that it has referenced the knowledge document but it has clearly not.
Can someone from OpenAI investigate this?

I really wish someone from OpenAI would comment on these issues. This is a major reason I am paying for use - Custom GPTs. All of mine that use uploaded knowledge documents are completely useless due to hallucinations.

Try to keep questions simple and get it to reorganize all the documents and rewrite. Save as pdf and the reupload. Hope it works!
