I have a project and a skill setup for a weekly repeated automated report. I tried unsuccessfully six times to get Codex to run this task (it was able to do this exactly one week ago without interruption). Here is how it summarized its own failure:
“I failed the main task: I provided no finished downloadable workbook despite your repeated instruction to continue until completion.”
I would expand this to include that I manually directed it how to complete the task, included the /skill that I built for it containing all the rules and hierarchy needed, provided it an example document, and had computer use enabled and reminded it where to go to run the report. It initially started but repeatedly stopped.
What am I doing wrong? Did something today change. I was running on the exact same model: Chat GPT 5.6 Terra (as last week). I had the same issue with another similar report but I actually made some headway and got it to finish that other one. On this one specifically, it repeatedly failed and I ended up manually completing on my own.
