both 4o and 4o mini have the same problem
I have the same problem as well. Seems to be a bug. Try loging into an app instead of using browser, worked for me there normally

I am having the same problems and it refuses to pull from the memory bank. I had to delete a story ive been working on for weeks because it refused to process it and generated nonsense. Im a plus user as well. It gotten crappy and crappy the more i use it. Ugh

Same here, just paid again, and both browser normal (private windows) clear cookies, and on phone browser and using win11 app. All fail its like ive paid for nothing. Not going to pay another month, if this continues, having to move to another AI is time consuming and painful.
Still doesn’t work… What happened, a bug or a policy change?
