Summary:
In Plan Mode, Codex’s markdown output intermittently fails to render correctly. The issue affects all browsers tested (Safari, Chrome, and ChatGPT Atlas). Once a broken render occurs in the first version, all subsequent versions are also broken, and there is no way to recover without reloading the entire plan (which doesn’t guarantee a fix).
Steps to Reproduce:
- Open Codex web interface.
- Enter Plan Mode.
- Generate a plan with multiple versions.
- Observe that Version 1 sometimes displays raw markdown or broken formatting.
- Switch to Version 2, 3, or 4 — all inherit the same rendering issue.
- Refreshing the browser, restarting the computer, or switching browsers does not resolve it.
Expected Behavior:
Markdown should render correctly in all plan versions with consistent formatting across browsers.
Actual Behavior:
Markdown renders as raw or malformed text. Formatting such as lists, bold text, inline code, and citations appear broken. The layout remains stuck across all versions.

Frequent (≈70–80% of sessions).
