400 Item 'rs_xxx' of type 'reasoning' was provided without its required following item

Hi,
400 Item ‘rs_0a3c51c0b1660ca30069967723b06c819983e34d2a6e015f83’ of type ‘reasoning’ was provided without its required following item.

I keep receiving this error. I tried to workout the issue, I thought initially its the model issue so I changed it to Chatpgt-5 but it persists. Also disabled all the hooks including memory but no joy. Any recommendations will be great
