Renaming a Codex task may cause it to disappear from the sidebar

I have encountered this issue with two separate Codex tasks in the macOS desktop app.

In both cases, I manually renamed the task. Later, the task disappeared from the sidebar and could not be found by searching for its subject:

  • One task was renamed after the conversation had started.
  • The other was named before the substantive conversation began.

The second task was successfully recovered and had not been deleted. Its complete conversation and file history were still present in the underlying task record. Its status appeared as notLoaded; opening it directly changed the status to idle and made the conversation accessible again.

Expected behavior

Renaming a task should update its title without affecting its visibility in the sidebar or search.

Actual behavior

After renaming, the task may disappear from normal navigation even though its underlying history still exists.

Impact

The user is given the impression that an important project conversation and its history have been permanently deleted.

Suspected trigger

Manually renaming a task. This has occurred with two separate tasks, but I have not yet performed a controlled reproduction.

Environment

  • ChatGPT desktop app, Codex mode
  • Version 26.715.31925 (build 5551)
  • macOS 26.5.1 (build 25F80)
