I don’t have an answer for your overall question, however, were those Stop Sequences entered prior to these submissions? Unless I’m misreading it looks to me like they were ignored.

Yes, the settings must have been changed after submission. That completion, of course, isn’t possible with a response length of 1 token (besides the stop sequences and penalties).
