A few days ago (More than 2, which is the longest they take), I filled out the OpenAI form to request an OpenAI app. but after waiting a few days, I still haven’t received an email back. Does this mean my app has been rejected or is it something else?

Have you happened to have checked your Spam by chance? Sometimes these messages do end up there.
