Every batch since 2026-08-28 13:09 UTC fails in validation with
“Cannot find file <input_file_id>, or organization org-HPYx… does not have access to it.” (27+ consecutive failures, request_counts.total = 0). Last successful batch was 2026-08-27 10:06 UTC. The input file is retrievable via GET /v1/files/{id} and /content (200, purpose=batch, status=processed). Reproduces on both /v1/images/edits and /v1/chat/completions, immediately after upload and 90 s later.
